2024-25 CYS Registration Form

Registration is open for Catholic Youth Swim Team
Please register your swimmers and divers. This registration system is to register for the CYS swim team, entering swim meets, and sending communications.
*All payments will go through Sacred Heart Cathedral School, $150 for the season

Information Sheet (schedule, practice times, meets, etc.) for the new season TBD once all schedules have been decided.

Athletic Fee: 6^th-8^th graders -$150, this includes all practices & meets fees, make checks to Sacred Heart School
($100 - 5^th graders who plan to participate in practices only, 5th graders may not compete)

2024-2025 Participation Forms

Important Participation Forms Needed for SHCS Athletics

In order to participate in SHCS athletics, all student athletes must complete the following four participation forms:

1) Physical Evaluation Form (dated after April 15, 2024),

2) Student Parent Concussion Form

3) Cardiac Awareness Form

Please complete these forms and email them to Jackie Bertucci at [email protected].
*If you have already filled these out for another sport at Sacred Heart you do not need to do another set.
# If you are from another school than Sacred Heart I need copies of your forms to house with the swim team files.
